# Churn risk review
Use this for one account or a ranked portfolio.
## Check the signals
Compare current and prior periods for adoption, progress toward stated outcomes,
support volume and severity, unresolved commitments, sponsor or champion change,
engagement, renewal timing, and direct customer statements. Cite the source and
date range for every signal.
## Separate fact from cause
For each risk state:
- observed change and baseline;
- likely explanations and evidence for each;
- what remains unknown;
- business impact if the risk is real;
- the smallest next check;
- owner and due date.
Rank as **urgent**, **review**, **watch**, or **insufficient evidence**. Do not
label an account at risk only because it is quiet or has support tickets.
## Action brief
Propose a recovery step tied to the customer's goal. Draft any message for
approval. Never invent sentiment, promise a product change or concession, or
send the outreach.
